Preceded in death by his parents, Richard Ballak Sr. and Amy Ballak (nee Fowlow); brother, Dana Ballak; many other dear family members.

Survived by his brother, Martin Ballak; uncle, Garth Daly; many dear cousins and friends.

Visitation Wednesday May 22, 2024 from 4:00pm to 7:00pm at O'NEIL FUNERAL HOME 1105 E. 9th St., Lockport.  Family and Friends to meet on Thursday May 23, 2024 at St. Dennis Church (1214 S.
